Required Credentials
for RNs and LPNs*
All applicant RNs and LPNs are screened to determine qualifications and competence as described in the current standards of the Joint Commission on Accreditation of Healthcare Organizations (JCAHO) and the Guidelines for Use of Supplemental Nursing Services as published by the American Nurses Association Commission on Nursing Services.

At least one year of experience in a Skilled nursing facility within the past three years.

Proof of compliance with applicable immigration laws and maintenance of I-9 documentation..
-
Current health history, including a record of immunizations (MMR & Varicella), current PPD skin test or Chest X-Ray / TB Mantoux Test , and Physical Examination.

A minimum of three (3) professional references of past or current employment.
-
Successful completion of pre-employment drug screening. Recent 12 panel drug screen.
-
Clearance of appropriate criminal background checks.
-
Recent CEU's.
-
Original and current driver license or State ID.
-
Original Social Security card.
-
Original and current Illinois State issued nursing license.
-
Original and current American Heart Association CPR/BLS for the healthcare provider.
-
Original and current fire and electrical safety training card..
-
Completed physical examination within the year including Snellen (Eye Exam).
-
TB skin test (PPD) within the year. If the results are positive, we require a CXR within 2 years. .
-
Immunization records for MMR, Varicella, DTAP, Hepatitis B and Hepatitis C.
-
Certificate or proof of mask fit testing.
-
ACLS for ICU TELE, DOU and ER RNs.
-
PALS for Peds , PICU and ER RNs.
-
NALS/NRP for NSY and NICU RN’s.
-
MAB for all nurses who work PSY and ER Units.
-
NRP and Fetal monitoring certificate for L&D RN’s.
-
Certificates of continuing education..
-
H1N1 and Seasonal Flu Vaccination of a Respiratory Mask Fit.
*Note: Some facilities may require/ if not additional credentials depending on Facility, Hospital or Patient need/s.
